In consideration of the services of the Maryland Freedom Athletics Program (MFA), their officers, agents, volunteers, participants, directors, employees, and all other persons or entities acting in any capacity on their behalf, I hereby agree to release and discharge MFA, on behalf of myself, my spouse, my children, my heirs, assigns, personal representatives and estate as follows: I acknowledge that the program requested entails known and anticipated risks which could result in physical or emotional injury, paralysis, death or damage to myself, my child, to property, or to third parties. I understand that such risk simply cannot be eliminated without jeopardizing the essential qualities of the activity. I also acknowledge that there are risks to this activity that are also unforeseeable. Furthermore, I understand and acknowledge that MFA seeks safety, but they are not infallible. I expressly agree and promise to accept and assume all of these risks as well as unforeseeable risks that might occur when I am and/or child is participating in this activity. My participation in this activity and/or my child’s participation in this activity is purely voluntary, and I elect to participate and/or have my child participate in spite of the risks, both known and unknown, foreseeable and unforeseeable. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless MFA from any and all claims, demands, or causes of action, which are in any way connected with my participation and/or my child’s participation in this activity or my use and / or my child’s use of MFA equipment, our own equipment, or MFA used facilities, including any such claims which allege negligent acts or omissions of MFA. Should MFA or anyone acting on their behalf be required to incur attorney’s fees and cost to enforce this agreement, I agree to indemnify and hold harmless them for all such fees and costs. I certify that I have adequate personal insurance to cover any injury or damage I sustain and/or my child. I further certify that I have no medical or physical condition, and/or my child has no medical or physical condition which could interfere with participation in this activity, or else I am willing to assume, and bear the costs of all risk that may be created, directly or indirectly, by any condition. By signing electronically accepting this document, I acknowledge that if I or my child sustains an injury and/or property is damaged during my or my child’s participation in this activity, I may be found by a court of law to have waived my right, my spouse’s right and my child’s right to maintain a lawsuit or claim against MFA because I have intended to release them by signing electronically accepting this document. I certify that all information provided is true and accurate and that any false information will result in my/my child's immediate removal from this program and loss of all registration fees associate with this registration. I further agree that if found guilty of falsifying this registration, I assume all financial responsibility for damages incurred to the program and MFA due to my negligence.  I have had sufficient opportunity to read this entire document and I understand it. In consideration of my child being permitted by MFA to participate in its activities and to use its equipment and facilities, I further agree to indemnify and hold harmless from all claims which are brought by and/or on behalf of Minor, by any entity, and which are in any way connected with such use or participation by Minor in the MFA activities. I also agree to all the terms and conditions of this entire agreement on behalf of my minor child and I further agree to be bound by its terms.  Parent/Participant also acknowledges and agrees (1) to respect the team’s coach and abide by his/her decisions for the team, to not coach the game from the sidelines nor subvert his/her authority in any way, and direct all issues or complaints to the MFA assigned representative: (2) to strictly adhere to MFA Code of Conduct and all Rules governing the use of School/County Facilities, to refrain from offensive comments to players, coaches, or officials. Spectators exhibiting disruptive behavior, or violating the Code of Conduct or rules will be required to leave the grounds immediately!; (3) to permit the use of participant’s/my child’s likeness (e.g. photos) and/or name in advertisements and literature and/or to be posted on the website for the above program; (4) to return all rented or borrowed equipment when notified to do so, the failure of which will result in the forfeiture of any deposit, and being barred from future registrations in MFA programs.

I will play by the rules and resolve conflicts without cheating or fighting. • I will treat other players, coaches, and officials with respect. • I will not yell at or tease a teammate for making a mistake or losing a competition. • I will not use profane (swearing) language or gestures. • I agree to put forth my best effort and, regardless of the outcome, I will know that I gave 100%. • I will listen to my parents and thank them for giving me the opportunity to participate in organized sports.
I will remember that children participate in sports for their enjoyment, not ours. • I will encourage our children to participate and not force them. • I will focus on our children's efforts and performance rather than winning or losing. • I will encourage children to play according to the rules and to settle disagreements without resorting to hostility or violence. • I will never ridicule or yell at a child for making a mistake or losing a competition. • I will remember that children learn best by example. I/We will appreciate good performances and skillful plays by all participants. • I will respect the rights, dignity and worth of every young person regardless of their gender, ability, cultural background or religion. • I will show appreciation and respect for volunteer coaches, officials and administrators. Without them, our children could not participate. • I will respect the decisions and judgments of the Referees and teach our children to do likewise. • I will never exhibit disruptive behavior, hostility or violence, and never use insulting, derogatory, threatening or hostile language towards players, officials, coaches or other fans. • I will support all efforts to remove verbal and physical abuse from sporting activities. • I will never bring, use or consume any alcohol or illegal drugs at any event. Field use is by permission of the Parks and Recreation Department. I will not jeopardize field use by my behavior. • I will promise to adhere to the values we all share such as excellence, honesty, respect, responsibility, fairness and compassion. • I will promise to act responsibly, set a good example, and at all times to demonstrate the practices and principles of good sportsmanship, which will reflect the dignity, integrity and enjoyment of the sport. • I will inform family and friends who attend events to abide by the above same rules • I will realize that games are just what the name suggests A GAME. • I will promise to stay behind the sideline barrier at all times. • I will respect fans, coaches, and participants. • I will work the concession stand, fields etc. because everyone is a volunteer. • I will ensure that arrangement are made to pick up my child after program events in a timely manner.
Booing or heckling an official’s decision. • Criticizing officials in any way; displaying temper with an official or Coach • Blaming losses on officials, coaches or other participants. • Using abusive language. • Ridiculing or yelling at a child (including your own) for making a mistake or losing a game.
Registrations are added on a per team need basis.  Upon completion of the registration request, you will be notified of the payment process for receiving your "Registration Raffle" tickets.  Payment must be received in full prior to equipment being issued and the player being added to the active roster.  Please note that there are NO REFUNDS for registration once a player particiaptes in a practice or games.
